To register for Catalyst mail order call 1-866-814-7105, go online to catalystrx.com and click on mail service or send the registration forms in via mail to the address provided on the form.
If registering by phone you will need your:
- Member ID number (on your prescription benefit card)
- Medication name
- Doctor name and phone number
- Shipping address
- Credit card and expiration date
