What is Disease Management?
Disease Management and what we refer to as Care Management are very similar. We try to stay away from describing what we do as Disease Management because our service is not limited to helping members with only certain conditions. Traditional Disease Management programs address conditions like diabetes, asthma, congestive heart failure and rheumatoid arthritis.
WellNet’s Care Management provides the resource of a Registered Nurse to assist and guide members with any concerns or challenges they have with managing their health. Care Management is designed to help members adhere to their prescribed treatment plan, make life style changes to improve their health and improve their understanding of their health conditions and prescribed medications. Registered Nurse Care Managers provide guidance by phone and through our secure online platform, P2Pme.
What is Wellness?
WellNet Interactive offers a wide array of wellness services designed to both identify members at risk for health conditions and to promote healthy behaviors. Services available include:
- Health Risk Assessments
- Wellness Seminars on topics such as stress management, smoking cessation, nutrition and Weight Management
- Coordination for biometric/ lab screenings
- On-site blood pressure screenings
- Health Fair coordination
- On-site Registered Nurses for 1:1 consultation
- On-site smoking cessation program
What is Case Management?
Traditional Case Management involves assessing members for specific needs for services and equipment, authorizing payment for those items and setting up the delivery of those services/equipment. There is also a degree of following a member to be sure the prescribed treatment plan is being followed. Typically, Case Management takes place post hospitalization or immediately after a new diagnosis, for example, cancer, a stroke, premature birth or an accident. WellNet Interactive Care Managers coordinate with Case Managers but we do not provide the same service.
